HealthPass by Faith Family Medical Center is a low-cost, membership-based solution for employers interested in providing their employees with affordable, high-quality and dependable healthcare – for a fraction of the cost of traditional healthcare plans.
For $30 per participating employee per month, HealthPass members have access to the following primary care services:
Four annual office or telehealth visits with an experienced Faith Family Medical Center provider or Behavioral Health Nurse Practitioner
One annual physical, including provider-advised labs
Faith Family Medical Center’s Journey to Wellness Program and group classes (such as cooking demonstrations and exercise classes)
Health information events related to conditions, such as Diabetes Day and Hypertension Day
An annual flu shot
HealthPass members also receive discounted services including:
- Additional office and telehealth visits, as well as counseling services
- Diagnostic exams, such as X-rays, CT scans, MRIs and ultrasounds*
- Referral access to community specialists*
- On-site pharmacy assistance
*For uninsured employees
