COST
Faith Family Medical Center utilizes a sliding scale based on income and number of dependents. Patients visits range from $25 and $60 per visit. We are a nonprofit organization. Patient fees cover only a small portion of our yearly budget; the balance is provided by grants and private donations from caring individuals, foundations, churches and businesses.
Please note: Additional fees, in addition to your office visit payment/copay, may be incurred during a patient visit. Your provider will review these with you prior to the service performed and you may decline if you so choose.

Faith Family Medical Center (FFMC) provides care to those who are working at least 20 hours per week, and their qualified family members, or a full-time student (12 hours of course-work.)
Underinsured:
Faith Family Medical Center (FFMC) provides care to those who are working at least 20 hours per week, and their qualified family members, or full time student (12 hours of course-work.) If you have insurance, your income must not exceed 400% of the federal poverty level. We are unable to see if you have Medicare, Medicaid (TennCare), or Tricare. (Chart below updated April 2023)

NEW PATIENT INFORMATION & PATIENT FORMS
Faith Family is not a walk-in clinic. All patients must have an appointment to receive care.
New patients need to bring the following information at the time of their first appointment:
- A current photo ID
- Proof of household income: acceptable forms include…
- One month of paycheck stubs (for patient and patient’s spouse if both working)
- A letter from your employer stating your pay rate, number of hours worked in a week, and how often paid. The letter must be on company letterhead with the employer’s contact information or notarized.
- Unemployment letter: Official unemployment letter stating amount received per week.
- Self-employed: MUST provide the following:
- Current tax return: First two pages of return that show Adjusted Gross Income, number of dependents, and Schedule C; AND
- 1-month personal deposit bank statement showing income OR invoices and receipts from customers showing you receive payments/income OR current year Business receipt book.
- Student: Registered Full Time (12+ hours). Must provide a physical copy of current class schedule each semester.
- Please also bring any current medical records, a list of your current medications, and medical release

Please review the WELCOME TO FAITH FAMILY MEDICAL CENTER patient letter. This has important information regarding income information, appointments, lab results, and phone calls from providers.
FFMC Nondiscrimination Policy:
Faith Family Medical Center provides quality, primary healthcare to working uninsured and other underserved individuals and families regardless of race, color, national origin, creed, religion, sex or background.
NO NARCOTICS POLICY
FFMC is committed to providing high quality medical care to our patients. We will continue to evaluate and treat pain experienced by our patients. We will offer conservative narcotic-free treatment of chronic pain which is associated with some medical conditions. There are multiple options for treating acute pain including oral medications, physical therapy, splinting, exercise, heat and cold, etc. which we can provide or refer our patients. At times, certain conditions will not be able to be treated in our clinic because we will not be able to provide services in a safe and effective method.
The treatment of chronic pain with narcotics has resulted in thousands of deaths, abuse, and diversion of narcotics into our schools and society. Tennessee is one of the states which has been the most severely affected. After evaluating the new guidelines for dispensing chronic narcotics, we have determined our clinic is not set up to and therefore will not manage chronic pain with narcotics.
In the case of a need or desire for chronic narcotic therapy, we recommend obtaining care from a clinic which specifically treats chronic pain. Patients who choose to pursue this option will still have the opportunity for their other medical problems to be treated by FFMC. FFMC will under no circumstances however provide any chronic narcotic therapy.
